Your first payment will include the enrollment fee and first month of service. Three months of commitment is required and will be due even if you choose to stop services prior to this. Complete the enrollment on-line and set up payment with bank account, debit or credit card. Continuity of care is best in order to obtain and maintain health and continued membership is preferred.
During any enrolled month, you can call me, text me, schedule a house call (those are an additional cost), or schedule to meet me at my office. I do require an in person visit with all local new patients. There is no co-pay or deductible for each of those visits with me.
